CONDITIONS OF APPLICATION FOR EMPLOYMENT
I certify that all of the information furnished on the Application is true, complete and correct. I understand and agree that any falsification, misrepresentation or omission of fact either on this
Application or during the pre-hire process will be reason for (1) my not being offered employment or (2) dismissal at any time from the service of EMP if employed.
I understand that nothing contained in this employment application or in the granting of an interview is intended to create an employment contract between EMP and me for either employment or for
providing of any benefit. No promises regarding employment have been made to me and I understand that no such promise or guarantee is binding upon EMP unless made in writing and signed by the president
If an employment relationship is established, I understand that my employment and compensation are for no definite period and may, regardless of the time and manner of payment of my wages and salary,
be terminated at any time by me or the Company, with or without cause, and without any previous notice. I also understand and agree that the Company has the right to unilaterally modify and/or terminate
any policies, practices, procedures and standards it has adopted or implemented, to the extent not limited by law. I acknowledge that no Company employee nor representative, other than the President, has
either the power or authority to enter into any agreement for employment for any specified period of time, or to make any representations or agreements contrary to any of the foregoing, unless that
agreement is in writing and signed by the President of the Company. I understand that any prior representations, promises, contracts or statements made by or on behalf of the Company are expressly superseded
by the foregoing.
I further agree that by submitting this application that any dispute concerning hiring and potential employment and actual employment can only be resolved through binding arbitration conducted by
the AAA using their rules that are in effect and must be initiated within 180 calendar days of the date of the alleged wrongful act or alleged discrimination.
As stated above, the Immigration Reform and Control Act of 1986 states that employers must require all persons hired to submit documents to the employer showing their identity and their right to be
lawfully employed in the United States. It also requires that the employee complete and sign a government form to this effect. I understand that if hired by EMP, I will need to furnish documents for
inspection that verify my identity and indicate that I am legally permitted to work in the United States. I understand that these documents must be provided within three (3) working days of employment. If
the original documents are not available, I agree to submit proof that I have applied for the required documents.
I understand that in the event I am offered employment by EMP, such employment is contingent upon my passing a drug urinalysis screening test. I hereby consent to provide a sample of my urine for
analysis and testing.
I have filled out this application, declaring the information is true and correct and I have read and understand the above conditions and agree to accept and abide by them. I hereby authorize my former
employers, schools, and all others to give any information they have regarding my employment/ records with them and I release them and their companies from any liability for any damage resulting therefrom. I
hereby affix my signature as verification of the foregoing statement.
